Dear Library Friends:

The Birthday/Gift Book Program is coordinated by the Falk School Library and the PTA. It provides the library with valuable support by expanding the library collection while commemorating the birthdays of students.

You may participate in several ways:

• You and your child/children may select book/s from the Gift Book List.  Indicate a first, second, and third choice by writing the titles in the "notes" section of the Gift Form.  Send a check in the amount of the price of your first choice, an average, or an amount of your choosing.
• Your child may choose a book/s from the other new library books. The number and selection varies during the year. Send a check in the amount of your choosing. (Most new books cost about $16.00.)
• Make a donation toward the purchase of other books. Feel free to contact Ms. Herward to discuss your choice.

Print the Gift Book Form (pdf file).  Fill in the requested information. Make checks payable to FALK SCHOOL LIBRARY FUND.  Send check and Gift Form to Ms. Herward by way of the school office.

A bookplate will be placed in each book unless otherwise directed. When possible, your child will be the first to borrow the book from the library. Gifts are usually acknowledged in FALKTalk.  If you do not wish to have this information publicized, note this on the Gift Form.

Remember the Falk School Library when making memorial contributions and at holiday gift time. Teachers appreciate library books donated in their name. If you would like to make a special donation or need additional information contact Ms. Herward. The library phone number is 412-624-8010.
